New Community Center Forms

As of 10/1/21, Elizabeth Township will require the following documents to be turned in to rent the Community Center.

(Special Requests Form IF APPLICABLE)

These documents will ensure that renters fully understand the responsibilities and requirements of renting our Community Center. If you are planning on renting the Community Center AFTER 10/1/21, please review these documents and turn them in when required.
